Oyster Fest 2015 food vendor application. Submit complete application with payments to the address listed below by APRIL 6, 2015

VENDOR FORMS:
  • City of Arcata One-Day Business License. Submit completed license application and a check made out to the City of Arcata (not Main Street). Send to Arcata Main Street.
  • Temporary Food Service Permit from the Humboldt County Health Dept.
  • Certificate of Liability Insurance naming Arcata Downtown Business Community as additional insured. (single day certificates can be obtained at www.galescreek.com)

Upon request, all forms and applications can be mailed to you, or you can stop by O’Fest Headqtrs at 839 9th Street, 2-5:30pm M-F, to pick them up.

Applications for other vendors, sponsors and artists will be online and mailed early March.
